Apple IIe Diagnostic Card

How do you use this? I have a platinum Apple IIe. What slot does the card go
in? And there's a SPST switch on the card. In one position, it doesn't seem
to do anything. When I switch it to the other position, then turn on the
Apple IIe, the screen shows a series of white vertical lines running down
the screen. I let the machine stay in this state for about 10 mins with no
change.
