Some further information for anyone who might kindly respond:

The problem seems to lie uniquely with Prodos 16/GSOS disks;
I've found no issue with Prodos 8 disks.  The few
"Fatal System Errors" that I've seen appear to reference
toolbox problems (?).  Setting the system/twgs speeds to normal
does not help.

The TWGS is the only card in the system (besides a 4Mb memory
card, which didn't change anything on removal); it's a ROM-03.

The TWGS self-test passes all tests, and the IIGS itself reports
(usually) a "System Good" on self-test (although occasionally
it throws a class 05012300 error).  I've tried the card in slots
3 and 4.  The only oddity that I can see is the TWGS oscillator,
which is 25Mhz-- aren't they usually at least 28?  It's a
rev1.5, with stock cache and all revA GALs.

Any hacks out there with suggestions?

Symptoms: Inability to completely boot a gs/os disk with either
an immediate freeze @ the progress bar screen, or a crash to
weird patterns and speaker blips, or a system fatal error.
