One possibility is that the disk is binding in the jacket. Try to
turn it by hand by pressing your fingers to the inside of the hole and
turning the jacket. If it doesn't turn easily, you can rub the edge
of the jacket against a table edge to loosen it up. (much easier to
demonstrate than describe, alas)
