Apple published a File Type Note describing the precise format of this
file type. It is called "FTN.42.XXXX".
I don't know whether the Apple II technical notes are still readily
available online (they used to be on ftp.apple.com, under the /dts/aii
directory). I could post it, but it is about 15K and is probably of
little interest to most people. If you can't locate a copy quickly,
