There are several possibilities. For instance ...
The PC side of ADT needs to run under DOS. (A DOS window under Windows will
not work-- you need to restart the system under DOS. Also, on the PC side, it
helps to locate the disk images you want to send in the same folder as
adt.exe .)
The baud rate needs to be the same for the Apple II and PC sides of ADT. The
COM port you choose (on the PC side) must be the one connected to the Apple
II.
For most setups, the connecting cable should be a NULL modem cable-- e.g. a
modem cable which includes a NULL modem adaptor.
For PC-->A2 transfers, ADT will correctly transfer only 143360-byte .dsk or
.po disk image files. (A .gz or .zip image file needs to be uncompressed
using a PC utility like WinZIP.) Also, the target diskette on the Apple II
must be formatted for DOS 3.3-- e.g. it should be a newly INITed disk.
For A2-->PC transfers, the 5.25" diskette you send must not be copy
protected.
Otherwise, more information about your setup would help:
o- What model Apple II and serial interface card (if any) are you using?
o- What version of ADT are you running? How did you obtain the Apple II part
of ADT?-- like, did you transfer it from your PC via a serial connection?
o- When connected via ADT, can you do a "D" ("Directory") command from the
Apple II and get a display of the PC directory on your Apple II?
