It is based on the original Mac FONT resource, but with changes. Most
of the header fields have the byte order swapped (little-endian instead
of big-endian), and there is an extra IIgs-specific header in front of
the Mac font header.
The whole thing is documented in the IIgs Toolbox Reference (QuickDraw
chapter, at the start of volume 2).

No, they are stored in the header, within the file. The font size and
family name are normally indicated in the filename, but this is only for
user convenience.

Back in the depths of time, there was a program called "Font Munger",
which converted a Mac bitmap font into a IIgs one. (I don't remember
whether this program ran on the Mac or IIgs.)
I also recall hearing of a completely different program on the Mac, also
called Font Munger, which had nothing to do with IIgs fonts.
