Universal Disk Controller.
The "Universal" here really only refers to drives released by Apple, or
100% compatible clones thereof. Third party drives with their own
controller card won't necessarily work with a UDC (for example, a
PC-compatible 5.25" or 3.5" drive which would work with a BlueDisk
card).
The name was appropriate at the time the card was released, but is no
longer accurate, since Apple's SuperDrive is not supported in at least
some of its modes, and the later version of the UDC dropped support for
the UniDisk 3.5 (but still supports the Apple 3.5 Drive and Macintosh
400K and 800K drives).

Can't answer that one. I have two: one is dead, but the other was
working prior to the host machine's power supply failing, and probably
still works.
