I sent IIGS user Trevor Mahan this URL containing lobotomy
instructions:

Trevor was a new IIGS user who did not have the UniDisk
driver and needed the 3.5 drive to work.  After performing the
lobotomy, GS/OS booted.

Could it be that you need to remove the UniDisk 3.5 driver
to avoid confusing the software?  That might explain why
it worked for Trevor but not for you.  If not, you may want
to send email to tjmahan@earthlink.net Trevor Mahan to
see what went differently.
