Get it here:
ftp://ftp.apple.com/Apple_Support_Area/Apple_Software_Updates/English-North_American/Apple_II/For_Macintosh
There is more information here:

I have just downloaded this software into my LCII. It unpacks into a Disk
Image which youi need either ShinkWrap or Disk Copy to mount on the
Desktop.
To access the card you have to run the emulator program.
Then the card tries to boot a non-existent ProDOS volume. That's as far as
I got with it.
You have to hold down the Control, Apple, and ESCape keys at the same time
to exit the Apple //e card.
I found I could not make the card access the Intenal Superdrive in the
machine. I could not create a ProDOOS partition on my HD either. This may
be because my LC II has a 100meg drive which cannot be formatted with
apple's drive set up.
I tried connecting a 5.25 drive from an Apple//c to the Apple //e card, but
it said UNABLE TO BOOT. Anybody got any comments?
If you can successfully create a ProDOS partition, then you can copy files
to it from the Mac partition using Apple File Exchange. AFE is on certain
System installer disks

Anybody who has got the ProDOS on their Mac LC II, please tell me how you
did it. I cannot use apple's HD Setup with my drive.
