Need Apple II SCSI Card rev C ROM image

I have an Apple II SCSI card (not the "fast" card) with the rev B ROM.  
I've been having problems trying to get it to work with GS-OS 6 and I hear 
I have to upgrade to rev C.  If someone could email me a 
hex/binary/S19/whatever image of the ROM, it would be trivial for me to 
re-burn it.

I did a web search and failed to find a copy of the ROM image.  If all 
else fails, I know someone who says he has a card with the rev C ROM.

I am presuming that the PAL chips do not need to be upgraded.  Is this correct?

Also, I presume that the eight pairs of jumper points select the card's 
SCSI address.  If this is so, is 8-9 the correct pair (the way I found the 
card), or is 1-16 the correct pair?
