You don't say if the 5.25" disks are properly notched. It may sound
like an insulting question, but have you done this with a disk notcher
or cut out notches in the proper places? Been so long since I had a
new 5.25" disk I'm not sure where my notcher is but you might try
this if you have not already.
Also, a good cleaning of the disk drives with a cleaning disk may help
if you have not already done that.
