Sorry I launched off on the P QuadBoard..

Quadram did have an Apple ][ on an 8 bit card for IBM PC's.  From my
experience this board was very tempermental and had to be installed in
an IBM PC only.  The board hooked itself into the floppy drive cable to
allow Apple ][ disks to be read/written.  If you find one of these
boards RUN! 

Central Point Software also used to sell the Copy ][ Option board which
worked very well in moving software between Apple ][ disks and the PC. 
Best I remember the board only worked for 5.25" drives even though it
would work in almost any ISA bus machine.  I have seen these for sale at
hamfests and computer flea markets here in Atlanta.
