Well, it looks pretty much like a clone of the original Microsoft
SoftCard.
Are you getting the hang when you turn the Apple on or when you try to
boot CP/M? Either way, I'd try it in slot 4 (preferred) or slot 5.
Don't worry about the switches, leave them all off.
The CP/M is version 2.2 with a custom Microsoft BIOS. It comes in two
flavors, one with a 40K TPA for 48k systems, and a 56K TPA for 64K
systems.
You should be able to find a copy of the Microsoft CP/M disks on Asimov.
