It is a Saturn 128k card from Titan Technologies. 

It works like the Apple Language card, but it has eight of these 12+4
kByte banks. In an Apple ][ plus system, you will use it in slot 0, with
a //e you can plug it into every slot, even slot 3.

Programs like Locksmith Fastcopy will recognize the whole 128k, most of
the other Programs only use one 16k bank.

CR1..CR3 indicate which bank is enabled, Read Enable means your
Applesoft ROM is disabled and the card is switched in.
