o Try booting with a different boot disk.
o Try cleaning the drive heads
o Consider checking the drive speed.  This can be done without a utility
if you take the case off of the drive and watch the strobe disk on the
drive motor on the bottom of the drive.  Trouble is, getting the mechanism
exposed in a DuoDisk is non-trivial, though not difficult.

Question:  Did you happen to interrupt a disk write operation with an
Apple-RESET or RESET with your boot disk in the drive?  I have had random
problems with DuoDisks trashing disks under those circumstances:  when the
machine restarts, the drive stays in WRITE mode, and trashes a cute spiral
as the head recalibrates for the boot.  Very messy, and the other reason I
don't much care for the DuoDisk (even though I have three of the darned
things).
