I had a very similar problem with my old Rev B RamFast.  It seemed to just
die.  I ended up pulling and re-seating *all* of the socketed chips on it. 
It works again now.

Also, if you haven't already, unplug and re-connect the power supply
connection to the motherboard.  This cleared up some intermittant problems
I was having.

FWIW, Sequential Systems didn't answer my email question either.
