comp.sys.apple2 Frequently Asked Questions (FAQ) at

questions you may have, including this one. Looking at section 8.10,
06xxxxxx is a failed serial test; your builtin serial ports may be
dead. Accelerators may cause the self test to fail; disable or remove
them should you have one.
