I'm not Nathan, but I can give at least a partial answer.

I've never seen a definitive list of bugs in the revision A and B ROMs.
There is some information in two places:

The "Apple II SCSI Card Technical Reference Manual".  I bought this from
APDA many years ago (part number APDA#K2B003).  I don't know if it is
available any more - it was replaced by the reference for the High-Speed
SCSI card.  Check with Byte Works.

This manual documents the Revision C firmware.  I can't see any obvious
places where it mentions differences between this and earlier versions.


There are two relevant technical notes from Apple:

SmartPort #5 (SCSI SmartPort Call Changes)

This mentions the change in control code 4 from FORMAT to the standard
SmartPort command EJECT.  (This is a serious problem with the revision A
and B ROMs: a badly written program that tries to issue SmartPort EJECT
calls to arbitrary devices can accidentally erase your hard drive!)

SmartPort #2 (SmartPort Calls Updated)

This mentions the same change.
