     The card you describe is not exactly a standard item. A company named 
"Vernier Systems" in Portland, Oregon used to specialize in projects and 
kits for connecting Apple ]['s to the 'outside world'. If they are still in 
business and still answer at 503-297-5317, they may have something close to 
what you want.

     An alternative is to contact a local Apple ][ user/hacker. It should not 
be too difficult to make the interface card and write a logging program. (If 
you cannot find a commercial supplier or a user/hacker interested in the card 
project, let me know.)
