Get rid of the CMS SCSI card and get either of these SCSI cards, which 
are trouble-free:

Apple Rev. C SCSI card

or

Apple hi-speed SCSI card.

The CMS card requires a CMS hard drive!

I have the MANUAL for CMS SCSI card, so if you want a photocopy of the 
whole thing, send $3 to cover copying and postage.
