Either/or/and:

1)  You're going to have to reformat the drive using the standard CMS
utilities, not the Mac utilities.  The card itself may have trouble
recognizing a drive which was not configured by that card.

2)  There is no such thing as a 40MB ProDOS drive, though you didn't
mention anything about partitioning above.  The maximum size a drive
can be is 32768 Kilobytes (32 Meg).

If your drive is set up as a ProDOS 40 Meg drive, this is VERY likely
the problem.
