You may be missing a software driver.  I believe the Videx (another 80-column
card for the ][+) and other 80-column cards needed drivers.

Getting lowercase in 40 columns requires replacing the character-generator chip
on the motherboard (I'm sure of this; my dad did it to our Apple ][+).
It doesn't sound like you did that, so I bet that your card gives lowercase in
80 columns only, and you need to solve the video problem to get lowercase.

Does the manual mention drivers?  Does it mention a disk (which you probably
don't have)?
