   Do you have a valid ProDOS disk in your 3.5" drive? If not, you may
want to consider inserting a 'sacrifice' disk you don't care about the
contents of. If there is a valid ProDOS disk, then there's probably a
hardware fault, ranging from dirty drive heads to a bad connection.

   Don't name your 3.5" disks /RAM5. That or try getting rid of any
Ramdisk.  This was before or after any ram expansion card faults?
