It's a 255 Grey Scale Video Digitizer, Circa 1979, The MicroWorks, Del Mar, CA.
I have one, and the book. I'm not sure about the software. It can be used as a
data analyzer too, or rather, be used to keep track of changes in the input
signal.
Take a look at http://www.hypermall.com/~tdiaz, there should be the screenshot
of that card in there.
I do not, at the moment remember an empty socket, but rather I do belive
there should be a Microcontroller in there, or something like a Z80.
