Can't help you with most of this, but...
: 2) when running a simple BASIC program, say to print 'Hello', the line #
: precedes the character string on the monitor screen i.e.

That indicates that Applesoft's TRACE function is active. Issue a NOTRACE
command to clear it. That's a debugging feature I find very unhelpful
'cause it interferes with embedded disk commands...
