Aside from obvious things like an unformatted or DOS-less disk, I had a 
similar problem that turned out to be the disk cable. One of the wires had a 
break inside. It occurred close to the card connector, and fixed it by laying 
the cable straight. I guess I've played with the cables/cards too much.
