Interesting Apple II Card



I need help identifying an interesting Apple II card.

Description:

Length is about 7 inches.  It has a 34 wire ribbon cable attached
to it.  25 of them go to a female DB25 connector, the rest go to
a little board that has 2 RCA jacks, and an RJ45 jack.  There is
also a cord coming directly from the card to this mini-board. 

There is no date on the card.  It does say:

ASSY 47704-000 REV, and
H870716 A.

The main chip in the middle of the board says JAPAN SED1341F OB.

Also, on the back it says "hpc-2-0".
