In the Apple publication About Your Enhanced Apple IIe: Programmer's Guide on 
page 5 mentions that [escape] [control]-Q will put you in 40 columns.  To go
to 80 then press [escape] [control]-R.  The note below table 1-2 on page 5 
says that these commands only will work with the video firmware on with PR#3 or 
from a program.
