Based on the label of the Eprom, I'd say this is a CPS card:
C)lock, P)arallel, S)erial multifunction board. It was (one of?) the
first multifunction boards to use "phantom slots", i.e. you plug the
board in one slot, and one of its functions appears in that slot, while
the other two functions are mapped to two other slots (and I believe that
you are not allowed to plug other boards into those other two slots).
Its serial interface is (if memory serves me) based on the Signetics (?)
2651 chip. There should be some way to connect a battery to run the clock.
There was (I think) some sort of jumper cable that you attach to the
motherboard's 74LS138 so that it can recognize the phantom slots.
