I had similar problems (or rather symptoms) and finally isolated it to some
NDA (don't remember which) that was trashing one of the system tools files
on the boot disk.  Reinstalling GS/OS, naturally cleared up the problem
until I used that NDA again.  Since removing that NDA the problem has not 
occurred.  Bear in mind, just doing a shift-boot had no effect since the
damage had already been done to the tool file.

Yeah, Ive seen this, too, but usually only when I get hasty with the three-
finger salute.
