The power cube that came with my Hayes 1200 Smartmodem has a connector like:

/ o \ I just measured approx. 12 volts AC (not DC) between the two
| o o | lower contacts and zero between the top contact and either
|_________| of the two bottom ones. That the top contact is ground is a
good guess, but it's not connected to the ground connector of
the 120-VAC plug-in.
