On the other hand, if the CPLD can be programmed from the Apple,

Modern CPLDs are normally programmed via a JTAG cable with just
4 dedicated pins connected. A parallel port cable can be built or
bought
for as little as $12.
In the case of my PCPI clone there is not much need to re-program
it, so I didn't bother to fan out the JTAG pins. Using Apple bus is
not
practical. Anyone likely to experiment with the CPLD logic will have
at least the JTAG cable and probably has enough hardware skills to
connect 4 wires.

Then a "flying lead" from the card's JTAG pins to the game port's
annunciator outputs and pushbutton inputs might do the job. ;-)
