Well, the joystick port is already 'outside' the case :-) If you want
access to the full range of IO pins on the internal gameport I'd
recommend:
1 16-way Insulation Displacement Header Plug
1 DA-15 Female Insulation Displacement socket
1 short length of 16-way IDC ribbon
In short, crimp the connectors to each end, plug the Header into the
internal game port, then mount the DA-15 on the rear panel. I use this
combination in combination with a Male DA-15 and another Header Plug
which I plug onto a standard 'breadboard' for prototyping. This avoids
the messiness associated with running cabling through the rear of an
Apple II. Be warned that IDC sockets tend to be rather 'fat' so if you
have a lot of cards installed space can get tight at the rear panel.
The advantage of this approach? No solder is required :-)

There are so many possibilities... I seem to recall there were a few
books back in the day that described simple interfacing projects. But
once you've decided what it is you're going to interface to there's
plenty of help available here to figure out the details.
