Back in the days, I've made a ton of 19 pin to 19 pin cables, by sawing 25 pin Canon connectors (ubiquitous). Of course you loose the shielding but that's mostly useless for this application.

It's funny that you mention that, since as I was typing that I was
thinking about how a "cut down" would work!

For an IDC connector, that would remove one end of the clamp, which
would be an issue, but one could certainly create a 19-pin body that
way, and use solder cup pins/sockets.
