OK I found the problem - it was the nut behind the wheel (ie. ME!). I did not realise, nor did I notice that my 5.25" drive had a floppy inserted and shared the same volume name - ie. "BLANK".
ADTPro must have been getting confused because there were two volumes named "BLANK", one of which was a 5.25" drive.
By the way, this has taken me several hours to figure out... I've been swapping drives, noting results, until I noticed the 5.25" drive shared the same volume name (I must have formatted it in ADTPro some time ago and forgot about it). Whoops!
I will disappear now and everyone will forget I ever mentioned it. ;-) cheers, -p
