Ok a couple others that you may or may not have tried to track down.

Dave Tribby looked to be very proactive in getting a copy back in the
day here on CSA2.  He posted a message:

About half way down there's a "Mark" who claimed to get everything for
v2.0.  I'm sure the university email is shot, but its a small lead if
anything.
