There is no doubt--this is *definitely* a monitor problem,
and it is the horizontal hold.

And, in light of subsequent information, my conclusion is
*definitely wrong*!  ;-)

Jeff nailed it when he noted that the raster is confined to
the normal Apple II area, since a horizontal hold problem
would usually go full-width due to overscan.
