Possibly dirty heads. Try cleaning them.
It could also be a broken wire in the cable - the read data line.

Sounds like the case and firmware don't match.
The 80-column card is a separate issue: an enhanced IIe has four
replacement chips on its logic board, but could have exactly the same
80-column text-only card that was originally available for the IIe (or
none at all).
It was only around the time of the late model "platinum" IIe that Apple
included the 64K card (miniature version) as a standard component. With
earlier computers the 64K card was a separate purchase. I don't recall
whether they always included the 80-column text card or whether it was
sold separately (around that time I only used the IIe lab at my school,
never bought one new).
