I found the tiny silkscreened label, and it is a Sequntial Systems card
(Rev 1)

The chip # is KM41C1001AZ-8. And it says Korea under that. Twenty pin
package. One jumper, currently off.

Pretty austere card in all. A pair of 7404s, a 74139, and a 74245. And
a pair of 220 ohm resistor packs. And the RAM chips of course :-)
