Oh yeah, it was common enough. Often a voucher for a disk exchange was included which for an additional shipping charge, you could get a 5.25. People usually complained that so vendors just started including both disks. It was an expense versus customer convenience issue. It became cheaper to put both disks in the box, instead of having inventory and shelf space wasted on two versions of the same product. Unfortunately, it took a little while to convince the greedier bastards. They thought providing 2 disks was like giving you two copies - one you'd keep, the other you'd give away to someone else with the right drive size.

When the Apple //c+ came out, many vendors (with Apple's help and marketing coop money to help defray transition expenses) went to the trouble of converting many of their titles to 3.5 format.
