Well, it's not true for all drives. I have a low-density 5.25" drive that works just fine when I flip a disk. I think some low-density drives do not work (I don't know which models) and no high-density drive works, AFAIK. I can post the exact model of my drive if anyone is interested.
Traditionally, at least for IBM style drives, the index hole is only during formatting. -- glen
