The pinout is documented in the Reference Manuals and the FAQs, IIRC.

16-pin IDC DIP headers used to be pretty easy to find, but now you
might have to order through a parts distributor.  An alternative that
is pretty easy to put together (particularly if you didn't want to use
ribbon cable) is to use a 16-pin milled pin socket as a header.

Just solder your cable to the appropriate pins on the "socket side",
secure the cable mechanically to the socket, and plug the "pin side"
into the 16-pin game port.
