I tried making an adaptor with the following pinout I generated from 
information in the Apple ][ Reference Manual and the FAQ:

D-9   I/O  Function
 1 <-> 3   Pushbutton 1
 2 <-> 1   +5V Power
 3 <-> 8   Ground
 4 <-> 7   Game Control 2
 5 <-> 6   Game Control 0
 6 <-> 4   Pushbutton 2
 7 <-> 2   Pushbutton 0
 8 <-> 10  Game Control 1
 9 <-> 11  Game Control 3

I found the 16 pin header at my local electronics store pretty easily.

It hasn't worked, but I haven't tested it extensively to see if it's bad 
soldering, and I only have one joystick, so I don't know if it has some 
problem.

I'd be interested to hear if you have success.
