In this context, "disconnected" means that the application has disabled
the ProDOS device driver for /RAM.  There is a documented procedure for
doing this (and for enabling it again afterwards).

To answer the implied other half of the question: on occasions where
Copy II+ doesn't show /RAM at all, this is probably caused by having
previously run an application which disconnects the /RAM volume (so that
it can use auxiliary memory) but then doesn't reconnect it again when
you quit.
