Sorry, made a mistake. It is a baseband video output. I've drawn a
schematic diagram of the card some time ago, and just took a look at it. 
You cannot adjust the video level, P3 (500 Ohm) sets only the modulator
input level.
Some cards have this VHF output, but not this type. The VHF modulator is
on the pcb, it is not an extra box. 

I can send you a copy of the schematic, it's around 200kB. The circuit
is very interesting, they use a TCA650 PAL decoder chip as an encoder.

The card gets /TEXT on pin 23, Video on pin 28, 14M on pin 24, ColorRef
on pin 35, Sync on pin 19, and +12V, Gnd. That means you do not
necessarily need a slot for the card. Doublesided tape and a couple of
wires will work, too.
