I have the same card with box and manual, currently buried in storage. It provides digital RGB on the DB15 connector (like that of the Apple Extended 80-column RGB card).
Regarding the two signals from J1: the left pin goes to the right side of R34 (at lower left of the motherboard), the right pin goes to to the left side of R7 (at upper right of the motherboard).
I can't remember what J2 (I think it was for color selection) or the dip switches are used for...
