There's a few causes for this:
Dirty Disk Drive (try swapping the drives around and cleaning them)
Drive needs speed adjustment (you can do this with many Apple II disk
utilities
but you need to boot the machine first)
Another very very common thing (although this is more common on the
later
I/O/Appledisk 5.25" controller) is the controller dying and exhibiting
that
behavior. It's almost always the same 74LSxxx chip that dies on the
card.
Seen this happen many many times.
