Which AppleWorks version is this?

Do you have a ZIP GS accelerator?  If so, this failure is normal.

To run the self-test properly, you have to disable the ZIP GS first:
turn the machine off, set switch 1-6 to the OFF position, then turn the
machine on again, holding down the Command and Option keys to start the
self-test immediately.

It should now pass without errors.  If not, then there may be a real
problem.  Let us know the result.

(Set switch 1-6 back to the ON position to enable the ZIP GS again.)
