Two possibilities spring to mind. If you have an Apple High Speed
SCSI card, you need to have a second hard disk on the chain to supply
termination power. If you have a memory card that has problems
supporting DMA, you need to turn off DMA on the High Speed SCSI.
You mention you have a 4MB memory card. Many older model RAM
cards, even if they are 4MB, do not support DMA. Try turning DMA
off and see what happens....but remember to add a second hard drive
to the chain one way or another.
