I think I've figured out the breakdown of levels, so to answer my
own question: There are indeed 90 levels total. The first 30 levels,
called Mission Zero ("Original Encounter") are unique to the Macintosh
port of the game.
As for the other 60 levels (missions 1 through 6) they are from
the original IBM PC version--mission 1 is actually the levels that
came included with the shareware version and mission 2, 3, 4, 5 and
6 are the levels you got in the registered version. In case anyone
is curious, the IBM scenarios should be 10 levels each (that does
not include secret levels though, you should be able to push at a
wall and find a hidden elevator behind it that takes you to a secret
level; there's a few scattered about all the scenarios). Oh, and yes,
you must load each scenario separately in order to play, it _won't_
automatically load them as you play through (you'll know when you've
completed a scenario by the special animation and music :).
Speaking of levels, if you manage to complete them all and want
to start searching for more, here is a web-page with dozens of home
made scenarios you can convert to use with the GS port of Wolf 3D:

many are compressed with a version of StuffIt which ShrinkIt GS 1.1
cannot open. Unless someone has recently written a better IIgs *.SIT
decompressor, you'll need access to a Mac or PC to open them).
